We attempt to schedule appointments at your convenience and when time is available. Since appointed times are reserved exclusively for each patient, we ask that you please notify our office 48 hours in advance of your scheduled cleaning appointment time if you are unable to keep your appointment. Patients who are scheduled for 2 hours or more are asked to give 1 week notice if possible, so that other patients who need our care can be scheduled if we have sufficient time to notify them. We realize that emergencies occur, but we ask for your assistance in this regard.
If you do not call to cancel and/or fail to show as scheduled, you may be charged a broken appointment fee of $50.00.
Payment is expected at the time services are rendered unless other arrangements have been made with our office staff in advance. Our office accepts Visa, MasterCard, (as well as debit cards which bear the Visa or MasterCard logos) Discover and American Express credit cards. Payment can also be made by cash, check, or money order. Please see dental financing section for extended payment options.
If utilizing your dental insurance benefits, benefit information must be current and available to the staff to verify prior to dental treatment. You are responsible upfront for any out of pocket costs associated with the terms of your dental plan.
Prior to treatment, we will provide an estimate of the treatment costs or co-pay. Usually the co-pay is a close estimate but in some cases an additional amount will be due. In the event extensive treatment plans are required, financial arrangements can be discussed.
We file dental insurance as a courtesy to our patients. We do not have a contract with your insurance company, only you do. Therefore, we are not responsible for how your insurance company handles its claims, or for what benefits they pay on a claim. We will assist you in estimating your portion of the cost of treatment, but we cannot guarantee what your insurance will or will not do with each claim. You are responsible for payment upfront if the dental insurance company pays you directly. We make every attempt to file claims accurately and to resolve errors if they occur. If you need an exact dollar amount a pre-authorization is required and that usually takes 6-8 weeks to receive from your dental insurance company.

If for any reason you decide to leave our practice, we understand that you have the right to request copies of your dental records/x-rays. We can provide the first copy to you free of charge, but there will be a charge of $25.00 for multiple record requests. We are required to keep the original record but will provide you with a copy if requested. All record requests will be provided within 1 week.
